This patient was diagnosed with with a blow-out fracture of the left orbital floor. Shortly after the initial presentation he did begin to develop double vision in up gaze. He also had a post-traumatic iritis, and an extra-macular choroidal rupture.
This patient did go to surgery for repair of the blow-out fracture which was confirmed upon radiologic studies. His choroidal rupture was simply monitored to make sure no retinal detachment or other complication developed. The iritis was treated with steroids and cycloplegia.
The post-operative results were quite good, with no subsequent diplopia. Good vision remained throughout the course. The iritis resolved nicely with treatment and the patient is currently under periodic follow-up to make sure no retinal detachment or neovascular changes develop secondary to the choroidal rupture.
