A 32 y/o man presented with complaints of poor vision in both eyes. He said that he'd never seen very well. His general appearance was rather poor. He had a disheveled look and was unshaven. A poor historian, he was forthcoming with very little personal or family history.
Visual acuity was 20/50- in both eyes uncorrected, with improvement to 20/30-2 in the right eye and 20/40+ in the left eye with a slight correction.
Muscle fields were normal. Pupils were normal. IOP was 19 mmHg and 21mmHg in the right and left eyes respectively.
Anterior segment biomicroscopy revealed the corneal appearance shown here. Both eyes were almost identical.
Both anterior chambers were normal; lenses were normal as well.
