This is a fascinating, classic case. The diagnosis shouldn't be hard. What I hope you'll get out if it are the spin-off conditions that make this case interesting.

C.B., a 73 y/o white male presented this spring and said that he was recently told at a VA hospital that he had a 'blood clot' in his left eye. He stated that he was experiencing a dark patch over the OS and had blurry vision in sunlight over the past two weeks.

Objective findings:

Best corrected vision: 20/25 OD, 20/80 OS

EOM: Full
AC: D/C
Lens: Mild cortical changes
IOP: 18 OD, 12 OS

Pupils/Iris: Normal neurologically, small tuft of vessels at 3:00 border in the left eye.

Retina: Normal OD, see photo for OS. Macula of OS was normal.

Question #1: What is this condition of the optic nerve head?

Question #2: Is the optic nerve head the only abnormal finding needing attention?

Question #3: What should management of this patient include?
