Upon further questioning, the patient did reveal that he had been hospitalized recently, but steadfastly held to his story that he had no idea why. Consultation with the patient's internist revealed that he had been hospitalized due to cryptococcal meningitis with increased intracranial pressure requiring multiple lumbar puncture. MRI ruled out any mass or other cause for the optic nerve edema. CT scan was also negative. The patient was again tested for Cryptococcal antigen, which was negative. The final decision by the various consulting doctors was to simply follow the patient over the next several weeks. Within two months, all edema was resolved and the patient was doing well.
